New Community Partner for Advance Care Planning: SATA CommHealth

On the morning of 1 August 2023, JurongHealth Campus signed an Advance Care Planning collaboration agreement with SATA CommHealth.

There are many collaborations between SATA CommHealth and public hospitals but this is the first time whereby staff from SATA CommHealth will be coming down in person to our campus to engage patients in ACP consultation. We are excited to have them onboard, with the unified goal of improving accessibility, timeliness, and care coordination in Advance Care Planning for our patients.
