The Case Management for At-Risk patients in the Emergency Department (CARED) programme, initiated in April 2021 by Dr Colin Ong, Senior Consultant, Emergency Medicine and Ms Kamala Velu, Assistant Nurse Clinician, Emergency Medicine, was a response to the critical bed shortage and Emergency Department overcrowding during the height of the COVID-19 pandemic.

The programme, designed to appropriately place frail and older patients and those returning to the Emergency Department within 30 days of hospital discharge, encompassed three key elements:

  • Early identification of at-risk patients

  • Geriatric assessments in the ED

  • Multidisciplinary case management

Collaborating with the Quality, Innovation and Improvement (QII) Department in both Quality Improvement and Health Services Research, the team secured funding from JurongHealth Fund to enhance and assess the programme's effectiveness in reducing hospital (re-)admissions and ED re-attendances. Dr Ong presented the programme's successful outcomes at the recent Singapore Health and Biomedical Congress in October 2023, earning him the prestigious Singapore Clinician Investigator Award (Silver) for his outstanding research.

Thank you Dr Ong and all staff who contributed to this programme!
